  1. IMPORTANT NOTICE The QMA now offers Group student insurance plan without proof of good health.

  2. The following coverage for medical students are now offered without medical proof : • $ 50 000 - life insurance • $ 50 000 - accidental deathand dismembermentinsurance • $ 1000 - disabilityinsurance with 90 days waiting period (1st et 2nd level) • $ 1500 - disabilityinsurance with 90 days waiting period (3rd and 4thlevel) • Premium has been waived : • Reduce disability rider • Cost of living adjustment rider • Future insurance option rider

  3. The annual premium : • 1st and 2nd level : $ 60 • 3rd and 4th level : $ 90

  4. Benefits will not be payable for a period of Total / Partial disability resulting directly or indirectly from a condition for which the student should have sought treatment or advise during the 12 months prior to the effective date of coverage. However, this limitation will not apply to any disability that begins after the coverage has been in effect for a period of 12 months from the effective date of coverage. Pre-existing conditions

  5. Coverage can be transferred to the Residency Plan No new proof or medical declaration to provide when transferring to our plans for residents and physicians.
