  3. Purpose • Provide high level product information to ensure general understanding • Communicate and review changes to products and processes. These changes are a result of: • 271 Workshop commitments and agreements • New product initiatives • New products and functionality in support of Statement of Generally Accepted Terms (SGAT) language and commission orders • Answer customer questions about product and process changes

General Product Overview • Collocation allows for the placing of equipment by the CLEC at Qwest’s Premises • The CLECs Collocated equipment is used to access unbundled network elements (UNEs), ancillary services and for Interconnection • Collocation includes the leasing to the CLEC of physical space in Qwest Premises • The CLEC also leases power, heating, ventilation and air conditioning (HVAC); and cabling in Qwest’s Premises • There are currently eight (8) standard types of Collocation available – Virtual, Caged Physical, Shared Caged Physical, Cageless Physical, Interconnection Distribution Frame Collocation, Adjacent Collocation, Common Area Splitter Collocation and Remote Collocation Customers requiring detailed product training should contact their account team representative.

Product & Process Changes Product Description • Global Changes • The following global changes have been made to the Collocation Product Catalog pursuant to SGAT language: • Wire Center and Central Office to “Qwest Premises” • Co-provider to “CLEC” • All Collocation • All other forms of Collocation other than the 8 listed can be requested through the Bona Fide Request Process • Qwest will permit CLECs to collocate any equipment necessary for Interconnection or access to Unbundled Network Elements • DSLAMs, ATMs and RSUs are included in permissible equipment for collocation • Direct Connection Point has been added to the types of demarcation points

Product & Process Changes Product Description • All Collocation Continued • References to Qwest Technical Publications have been omitted and replaced by NEBS Level 1 Safety Standards • Qwest shall not impose safety and engineering requirements on the CLEC that are more stringent than the safety or engineering requirements Qwest imposes on its own equipment located on its Premises • When adjoining collocation space is not available, Qwest will engineer a route for the CLEC to provide facilities between its non-adjoining Collocation spaces as a part of the Collocation order • DS1 terminations are available in increments of one (1) • SGAT language regarding reclamation and reconditioning of collocation space has been added to the PCAT • Effective September 6, 2001

Product & Process Changes Product Description • Physical Collocation • Physical Caged, Physical Cageless and Physical Shared Caged Collocation are now referred to as Caged Physical, Cageless Physical and Shared Caged Physical Collocation • Caged Physical Collocation • The maximum 400 square foot requirement has been removed for Caged Physical Collocation • Cageless Physical Collocation • The minimum bay size requirement may be reduced as smaller industry standard equipment bays become available • Shared Caged Physical Collocation • Charges between two parties can be prorated and billed directly to each CLEC • Common Area Splitter Collocation • Collocation was augmented to align with new SGAT language

Product & Process Changes Product Description • ICDF Collocation • The ICDF Collocation product description in the Collocation Product Catalog now includes fiber distribution panel as a type of frame used for ICDF Collocation • Adjacent Collocation • The Adjacent Collocation product description has been further developed according to the SGAT • The changes to this product offering will be effective beginning September 6, 2001 • CLEC-to-CLEC Connections • The CLEC-to-CLEC product description has been rewritten to be consistent with SGAT language • The CLEC-to-CLEC Cross-Connections product description has been modified to be consistent with SGAT language • If a CLEC disagrees with Qwest’s selection of a cross-connection frame the CLEC may request use of an alternative frame or arrangement

Product & Process Changes Product Description • Virtual to Cageless Conversions • Virtual to Cageless Conversion Product has been developed • Collocation Conversion product requests (except for Virtual to Cageless) will be considered on an Individual Case Basis • Virtual to Cageless Conversions involving billing and administrative system changes only will be completed within thirty (30) calendar days • This product change will be effective beginning September 6, 2001 • Collocation Reservation and Space Option • The Collocation Reservation and Space Option product has been added to Qwest’s product line • Effective September 6, 2001 • Entrance Facilities • Descriptions for Collocation entrance facility options have been added • Microwave Entrance Facilities • Microwave Entrance facilities standard product offering has been developed • Effective September 6, 2001

Product & Process Changes Product Description • Space Availability Report • In addition to the existing information provided, the Space Availability Report will now include: power availability, number of CLEC in queue for the specific Central Office, DS3 capability, and number of space reservations • This report was previously referred to as the Space Inquiry Report in the Collocation Product Catalog • Out of Space Procedures • Language concerning Space Denial Queue has been modified in the “Out of Space Procedures” document located within the Collocation Product Catalog • Guided tours now include a review of space reservations and space options • If Qwest incorrectly denies space, the original application date will be used to determine RFS, unless otherwise agreed upon by both parties

  11. Product & Process Changes Product Description • Early Access • The documented Early Access Procedures have been modified. In order to obtain early access into its collocation site, a CLEC must pay 80% of the remaining 50% of the quoted nonrecurring charges • Qwest Premises Earthquake Ratings • A Qwest Premises Earthquake Ratings spreadsheet is now downloadable from the Collocation Product Catalog in the Product Description Section • Product Implementation Notifications • Several product implementation notifications previously available in the Collocation PCAT have been reviewed and updated to align with the SGAT language. They Include: • SICM Guidelines • CLEC and Qwest Cabling Receipt Process • Collocation Completion and Billing Process • Express-Shared Fiber Process • Parallel Processing

Product & Process Changes Pricing • All Collocation • The rate element for collocation terminations may be requested in Shared Access and Direct Connection Configurations • The Space Availability Report charge language was added to the rate element section of the Collocation Product Catalog • CLEC-to-CLEC Connection charge language was added to the rate element section of the Collocation Product Catalog • Physical Collocation • The language describing the Space Construction and Site Preparation rate element for Physical Collocation has been augmented to align with the SGAT description • The Floor Space Lease rate element for Physical Collocation now includes a pro-rata share of the bi-weekly housekeeping services of the common areas in the particular Qwest Premises • Adjacent Collocation • Rate elements for Adjacent Collocation will be developed on an individual case basis • Effective September 6, 2001

  13. Product & Process Changes Features and Benefits • A feature and Benefits Section has been added to the PCAT • It outlines the benefits for each type of Collocation product offering

Product & Process Changes Implementation • Pre Ordering • Additional information including the number of CLECs in queue, DS3 capability, estimated completion date of power additions, and addresses of the remote premises that have been inventoried for remote collocation have been added to the Space Denial Report (previously known as the Space Availability Report) that is posted on the Internet • The forecasting procedures state the forecast needs to correctly identify floor space requirements and power requirements within twenty (20) percent of the forecast • If at the time of application, the forecasted type of collocation is not available the CLEC may specify a different type of collocation without affecting the collocation intervals • Forecasting procedures have been added resulting in shortened intervals with forecasting.

  15. Product & Process Changes Implementation • Ordering • Language has been added to the Collocation Product Catalog describing the application and quote acceptance stages of the ordering process • Language has been added describing the maximum number of applications that can be submitted per week per state by a CLEC that may affect standard installation intervals • Provisioning • Collocation Product Catalog Provisioning Section includes a link to the updated Service Interval Guide

  16. Product & Process Changes Implementation • Maintenance • CLEC shall not be responsible for any costs or charges incurred in the maintenance and/or repair of CLEC’s virtually collocated equipment where such costs or charges result from Qwest’s fault or negligence • Qwest will provide the CLEC written notice at least five (5) days prior to scheduled non-emergency power work • Qwest will notify the CLEC via the Abnormal Conditions Report (ACR) of general power outages and emergency power disruptions no later than thirty (30) minutes after the incident occurs • If an alarm condition exists, the CLEC will immediately be notified • Effective September 6, 2001 • CLEC with a Virtual Collocation site will have access to the demarcation point • CLEC may request to have circuit card(s) added, upgraded or replaced by calling the Qwest Account Maintenance Support Center

  17. Product & Process Changes Implementation • Billing • The activation of CLEC transport services and/or UNEs or ancillary services is coincident with completion of the Collocation and receipt of final payment • A walk through must be scheduled and performed no later than 21 calendar days following RFS
