  1. Intel International Science and Engineering Fair 2007 Required Forms

  2. Checklist for Adult Sponsor (1) • Print your first and last name legibly in blue or black ink. • Print your project title legibly in blue or black ink in the following format: The Effect of (IV) on (DV) • Place a check in boxes 1, 2, and 3. • If your project involves humans, place a check in the appropriate box under #4.

  3. If your project involves humans, you will also need to check ALL boxes under “Humans”. • If your project involves vertebrate animals, place a check in the appropriate box under #4. • If your project involves vertebrate animals, you will also need to check ALL boxes under “Vertebrate Animals”. • Place a check in the top four boxes under #5.

  4. If your project involves hazardous chemicals, activities, or devices, check “Hazardous Chemicals, Activities, and Devices” box and the first box below. • Note: Any chemical, including household chemicals, containing warning labels are considered hazardous.

  5. Student Checklist (1A) • Print legibly your first and last name in blue or black ink. • Print the number 7 for “Grade”. • If you have and would like to include your email address, you may do so in the appropriate place. • Print legibly your telephone number including area code in blue or black ink.

  6. Print the title of your project just as you did on Form (1). • For school, print legibly Challenger K-8. • The school’s telephone number is (352) 797-7024. • The school’s address is: 13400 Elgin Blvd., Spring Hill, FL 34609 • Your Adult Sponsor is Shone Jung, or yours science teacher. • Phone: (352) 797-7024 x 262 • For #5, check “No”.

  7. Projected Start Date: 10/22/07 • Projected End Date: 11/30/07 • Leave “ACTUAL” Dates blank for now!  • For #7, place a check in the “School” and “Home” boxes. • In #8, the blanks on the left should be completed as follows: Challenger K-8 13400 Elgin Blvd. Spring Hill, FL 34609 (352) 797-7024

  8. In #8, the blanks on the right should be completed as follows: (Your last name) Residence Complete Street Address City, State, Zip code Telephone Number including Area Code

  9. Research Plan • The question is your investigative question. • Your hypothesis should be written using “If…, then…because” format. • Your description should include a the following: the step-by-step procedure and a bibliography of atleast 5 sources.

  10. Guidelines for creating a bibliography can be found on the “Guide for Citing Sources” on my website. • If your project involves one of the areas outlined in #1-4, be sure to read and follow ALL guidelines for submitting your Research Plan.

  11. Approval Form (1B) • Place a check in both boxes below “Student Acknowledgement”. • Then,in blue or black ink legibly print, sign, and date. • You must also have your parent/guardian legibly print, sign, and date in blue or black ink . • Do NOT complete anything else on this page! 

  12. Qualified Scientist Form (2) • If your project involves humans or vertebrate animals, you will also need to have the doctor or veterinarian complete Form (2). • Some projects involving hazardous chemicals, activities and devices may also require a Qualified Scientist. If you are unsure, ask your teacher.

  13. Risk Assessment Form (3) • This form MUST be completed if your project involves hazardous chemicals, activities or devices. • If you must complete this form, the “Designated Supervisor” will be whoever will be overseeing the experimentation. It may be a parent or guardian. Most likely, this will NOT be your teacher! • The Designated Supervisor MUST print, sign, and date accepting responsibility.

  14. Human Subjects Form (4) • If your project involves humans, you MUST work with your Qualified Scientist (doctor/nurse) to complete the three questions at the top of this form. • The Qualified Scientist MUST fill in their name and telephone number below #3.

  15. Vertebrate Animal Form (5A) • If your project involves vertebrate animals, you must complete questions 1-3 at the top of Form 5A. • The Veterinarian overseeing your project MUST complete the box in the lower left-hand corner of this form.

  16. Other Information • If your project involves any chemical, including household chemicals, containing warning labels, you MUST attach appropriate MSDS forms for the chemicals being used. • MSDS forms may be accessed at the following websites: http://www.flinnsci.com http://www.hhmi.org/about/labsafe/lcss.htmlhttp://www.ilpi.com/msds/index.html

(This is also linked to the Science Fair page of my website.) • At this website, you can type directly on the forms. However, you cannot save your information. So, when you have completed the form, you MUST print it immediately. • ALL completed forms are due Monday, October 15!
