Heart Smart Sisters • Empowering women in ethnic communities to make choices that can help reduce their risk of heart disease. • The Heart Smart Sisters program is a series of classes designed to educate women about the causes of heart disease, the benefits of healthy diet and the importance of regular exercise. • Participants set personal weight-loss goals, take on healthy-living assignments, and track their progress at weekly check-ins. • The meetings also provide a chance for participants to develop a strong support system within their communities.   12. New Jersey Healthcare Quality Institute (NJHCQI) • New Jersey Healthcare Quality Institute (NJHCQI) was awarded a $550,000 partnership grant from United Health Foundation to support its “Healthy Communities Create Healthy Citizens” project. Presentation Name

New Jersey Healthcare Quality Institute (NJHCQI) • United Health Foundation’s support will enable NJHCQI to help local organizations expand innovative healthy-lifestyle and health-literacy programs in Trenton, Jersey City, and Cumberland County. • In Trenton, the grant will support health topic seminars at local food pantries and churches, where residents can “shop” at the food pantry, eat a healthy free lunch, learn about diabetes prevention and participate in fitness-related giveaways and receive helpful reading materials. • In Cumberland County, the grant will support a “healthy corner-store” initiative, where 27 corner stores will provide health screenings and commit to selling healthy food. Veterans Health Navigators • United Health Foundation made a $1.35 million grant to the Health Research and Educational Trust (a subsidiary of the New Jersey Hospital Association) • Will hire trained veterans to lead a peer-to-peer network to promote integrated care, including physical, mental, and behavioral health, and incorporate access to supportive social services that address the social determinants of health. Partnership with Camden Coalition of Healthcare Providers • In 2011, UnitedHealthcare and the Coalition began a partnership by sharing information among providers in- and outside of Camden and aggressive, face-to-face care coordination with members.  • Led to: • lower costs • more appointments with PCPs • fewer visits to specialists • fewer prescriptions at a lower total cost • fewer hospitalizations. Due to that success, the partnership expanded, and UnitedHealthcare now manages 8,800 members jointly with the Coalition and has added similar partnerships in Camden County (with CamCare) and Cumberland County (with Complete Care).
