STEM Ambassador There's a lot more compulsory information needed for Disclosures now you will need to bring the following on the 14th Dec: • Passport (If applicable) • Driving licence (If applicable) • National identity card (If applicable) • Proof of current address e.g. a utility bill or bank statement dated within the last 3 months • Passport sized photo of themselves • You also need your NI number and full 5 year address history on the form.
