  4. 2001 DM 2010 UGI bleeding , BUN/Cre elevated persistent renal insufficiency underwent left AV fistula creation Jan 2012 started hemodialysis July 2012 Went to ER for fever and cough with yellowish sputum Oct 2012 pneumonia Oct 17
