  2. Competency 1 • A patient was referred to our department with a small, tender and swollen lump on the anterior aspect of her left ankle. The referral queried a ganglion or foreign body. • When asked, the patient said she had had the lump for about a month and it had settled a little with antibiotics. She wasn’t sure, but said it might be related to a bike accident she had about 2 months ago. • It transpired that she had been mountain bike riding and dropped the muddy bike on her foot, where the cog had pierced the skin.
