Data and indicators: population health related issues that lead to chronic disease Aboriginal population of NSW 229,951 Aboriginal people live in NSW, 3.0% of the NSW population. 30.8% of the Aboriginal and Torres Strait Islander population of Australia reside in NSW. Tobacco 34.9% of Aboriginal adults in NSW are current smokers, compared to 12.9% of non-Aboriginal adults. 45.2% of Aboriginal mothers reported smoking during pregnancy compared to 7.8% of non-Aboriginal mothers. Smoking rate during pregnancy by Aboriginal women has been steadily decreasing and early indications are that NSW Health is on track to meet the NSW Health Plan target. Overweight and obesity 57.5% of Aboriginal adults in NSW are overweight or obese, compared to 52.3% of non-Aboriginal adults.

Data and indicators: population health related issues that lead to chronic disease Alcohol 28% of Indigenous Australians were more likely to abstain from drinking alcohol compared to 22% of non- Indigenous Australians. 40.1% of Aboriginal adults in NSW consumed alcohol at levels posing lifetime risk to health, compared to 25.5% of non-Aboriginal adults. Diabetes or high blood glucose 17.2% of Aboriginal adults in NSW had diabetes or high blood glucose, compared to 8.5% of non-Aboriginal adults. Circulatory conditions 1.7 times higher hospitalisation rate for circulatory diseases among Aboriginal people in NSW (2829.3 per 100,000 population) compared with non-Aboriginal population (1677.1 per 100,000 population). Blood-borne viruses and sexually transmissible infections Hepatitis C is almost five times higher and hepatitis B is about two times higher than in the non-Indigenous population in Australia. chlamydia is three times higher, gonorrhoea is about double while HIV is the same for non-Indigenous population in Australia.

Population Preventive Health Programs: Reducing Chronic DiseasesPositively impacting on wellbeing and mental health NSW Health has a variety of statewide population preventive health programs, as well as capacity building programs , which services can access for patients and staff. For some Aboriginal people, accessing mental health services may be the only interaction they have with the health system. There are also links between chronic diseases, population health issues such as smoking, diet and exercise; and mental health. Mental health services are therefore vital in promoting and linking consumers with population health programs and services, to adequately provide multiple and more holistic services to their consumers. The following slides will showcase the statewide population health programs available.
