The median age of the cohort was 67(31-88). Prognosis was worse in men with rs9989661 AA than AG/GG. At 3-years probability of DFS was 39%(95%CI 17-60%) vs. 56% (95%CI 45-66%) and OS was 44% (95%CI 21-67%) vs. 66% (95%CI 56-77%) in patient harboring AA vs AG/GG group (P= 0.03 & 0.039 respectively). Median survival was not reached in the AG/GG group; in the AA group median DFS was 1 yr and OS 2ys. In the multivariate analysis adjusting for stage, age, and ACT, males carrying rs9989661 AA had increased risk of disease recurrence (HR 4.37 95%CI 2.08-9.18, p < 0.001) and dying (HR 3.69 95%CI 1.65-8.24 p = 0.002) compared to those with AG/GG (HR=1, reference). There was no statisticaly significant finding in females. LMTK3 is a modulator of estrogen receptor (ER)α. Its level of expression and its germlinepolymorphism is prognostic and predictive of estrogen resistance in breast cancer.(1,2) Epidemiologic studies support that estrogen exposure is protective against intestinal type gastric cancer. Furthermore expression of ERα and lack of expression of ERβ are poor prognostic markers in gastric adenocarcinoma (GAC).(3) We have previously demonstrated that rs9989661(r9) AA allele is associated with shorter time to recurrence in non-metastatic GAC only in females in a US cohort of patients.(4) GAC has diverse pathologic pathways and risk factors in different racial backgrounds and geographic areas. Additionally, the allelic frequency of LMTK3 in Japanese population is different. We intended to verify the prognostic value of LMTK3 and its polymorphism in a cohort of Japanese patients with GAC. Methods We obtained either blood or FFPE tissue specimens from 169 localized (stage Ib-IV) GAC patients (60 females &109 males). Genomic DNA was extracted using the QIAmp kit. All samples were analyzed by means of PCR-based direct DNA- sequencing. All patients were treated with D2 surgery with or without adjuvant fluoropyrimidine based Chemotherapy at Fukushima Red Cross and Kitasato University East Hospitals. The median follow-up were 4 years. The primary and secondary endpoints of the study were disease-free survival (DFS) and overall survival (OS). To evaluate prognostic value of this polymorphism, endpoints were estimated using Kaplan-Meier method and compared by log-rank test. The level of significance was set to p < 0.05, and all statistical tests were two-sided. Multivariate analysis was performed to test the interaction between polymorphism and gender adjusting for other variables. All statistical cells performed using the SAS statistical package version 9.2 (SAS Institute, Cary, NC). Conclusions These results confirm our previous findings that LMTK3 is an independent prognostic factor for localized GAC. Interestingly the relationship between gender and prognostic significance is the opposite in Japanese cohort. The gender disparity can be due to the differences in the etiology (histological subtypes), management strategies, allele frequency, and degree of estrogen exposure in the two populations.
