  3. What do we do ? • Assist in linking newly diagnosed clients into care • Locate, re-engage and link out of care /lost to care clients • Identify and address client barriers to care • Verify Care Status • Make appropriate client referrals

  4. Referral Process • North Carolina Electronic Disease Surveillance System (NCEDSS) Referrals from Disease Intervention Specialist (DIS) Referrals from SPBC Prison Releasee Referrals from SBC Regions / SBC Transfer
