Plan for Continuous Quality Improvement, FY 13-14Templates for Local Health JurisdictionsThe following pages contain the templates that Local Health Jurisdictions may complete for their CQI Plan. Sites may contact the CHVP Quality Assurance Teams or CA-MCAH-HomeVisiting@cdph.ca.gov for assistance completing these plans. The chart-based format is intended to minimize the work necessary to develop a CQI Plan by reducing the actual writing and narration. The CQI plan demonstrates that the CQI process is site-driven and CHVP supported, and provides an opportunity for the CHVP QA Teams and sites to initiate a strength-based CQI process explicitly based on site needs and interests.Sites that wish to submit an alternate plan format should review the document “Required Components of a CQI Plan,” also available on the CHVP website and from the sources listed above.
