    1. PROGNOSTIC CONSIDERATIONS IN THYROID NEOPLASTIC LESIONS Prof. Dr. M.N.AKIL Department of Pathology Faculty of Medicine University of Aleppo

    2. THYROID NEOPLASTIC LESIONS Tumors of Thyroid Follicular Epithelium 1. Follicular adenoma, including Hrthle cell adenoma 2. Follicular carcinoma, including Hrthle cell carcinoma 3. Papillary carcinoma 4. Poorly differentiated thyroid carcinoma, including insular carcinoma. 5. Undifferentiated (anaplastic) carcinoma 6. Rare tumor types: columnar cell carcinoma, mucoepidermoid carcinoma, sclerosing mucoepidermoid carcinoma with eosinophilia, mucinous carcinoma.

    3. THYROID NEOPLASTIC LESIONS Tumors Showing C-Cell or Simultaneous Follicular and C-Cell Differentiation 1. Medullary carcinoma. 2. Mixed follicular-parafollicular carcinoma (differentiated thyroid carcinoma, intermediate type). Papillary Carcinoma 70% to 85% of all cases. Follicular Carcinoma 5% to 10% of all cases.

    4. THYROID NEOPLASTIC LESIONS TNM (tumor-node-metastasis) staging system is the most widely used system for staging thyroid cancer .

    5. Primary Tumor (T) TX Primary tumor cannot be assessed T0 No evidence of primary tumor T1 Tumor =2 cm, limited to the thyroid T2 Tumor >2 cm but =4 cm, limited to the thyroid T3 Tumor >4 cm, limited to the thyroid; or tumor of any size with minimal extrathyroidal extension (e.g., extension to sternohyoid muscle or perithryoid soft tissue). T4a Tumor of any size extending beyond the thyroid capsule to invade subcutaneous tissues, larynx, trachea, esophagus, or recurrent laryngeal nerve T4b Tumor invades prevertebral fascia or mediastinal vessels or encases carotid artery All undifferentiated (anaplastic) carcinomas are considered T4: T4a Intrathyroid (any size), and surgically resectable T4b Extrathyroidal (any size), and surgically unresectable
