  2. Mission Statement The mission of Love Your Heart Program is to optimize the health, safety and comfort of our patients, their families and the community. Mission Statement GOAL Our goal is to reach the mission by providing services in the best and most sensitive professional manner.

  3. Statistics of Santo Domingo Pueblo • Santo Domingo Pueblo is located 35 miles north of Albuquerque and 35 miles south of Santa Fe in Central New Mexico. Santo Domingo is one of 19 Pueblos in New Mexico with a population of approximately 4,000 (2002) members. • Santo Domingo Pueblo is deeply rooted in its tribal culture. As a member of the Keres speaking Language, 98% of pueblo members fluently speak their first language.

  5. Barriers To Success • About 1 out of 3 participants will usually need help with reading materials translated to Keres language for better understanding of diabetes information. The community of Santo Domingo Pueblo have health care services provided by Indian Health Service (IHS). Santo Domingo Health Clinic (SDHC) is located 15 miles away from the Pueblo and Santa Fe Indian Hospital located 34 miles north of the pueblo. Most participants in our program are beginning to transfer Diabetes care and services to the local health clinic. A majority of LYHP participants now have their diabetes check-ups at SDHC or at the LYHP office. Problems arise when participants do not fully understand the none keres speaking providers (Example: when primary care provider gives participant Diabetes education. When consulted by pharmacist with Rx participants may not fully understand instruction of DM prescription).

Our Response • To better serve our community, LYHP Staff are committed to do the best through: • Translation of health information. • 5 Love your Heart Program staff who are fluent in Keres provide one to one or group classes with participants. • Learning the language by interacting with participants because together we learn more.

  8. Love Your Heart Program • Currently we have 67 participants enrolled. 55 participants active. • Men/Women Ratio (Graph Below) • Age Groups (50% above 60 years old)

  9. Success! • Completion of HGHH curriculum for 16 participants. • To make it convenient for participants who can not make it to group HGHH classes we offer classes to individuals at home or offer individual make up sessions.

Purpose: To help get to know one another name (Indian Name) • Seating Pattern: Everyone will be seated in a circle. • Number of Participants: 10 people or more • Process: • The facilitator stands inside a circle, Have everyone in the circle say his/her indian name (remember if a participant does not have an Indian name give him/her a name) after each participant has a name, the leader points to someone in the circle and says “whose sitting on the right or left of you” the person has to say the participants name, if that person does not get name correct he/she stands in the middle, asking whose on the right/left side of them (we will be speaking in KERES or their Language that is spoken) • If it appears that participants know the name of the person to the right/left the leader may change seating. All participants must get up and find another seat, who ever is left standing re-starts the game.
