  1. Corporate Card Application Guide • Complete the ‘AMEX Corporate Card Application Form’ via Amex Singapore web link athttp://corp.americanexpress.com/gcs/clients/hp/region/docs/japa/SingaporeCardApp.pdf • Section 1 guidelines: • Leave the Name of Company To Appear On Card field blank • Complete the rest of the fields in full • Section 2 guidelines: • Indicate your employee No. (This is mandatory) • HP employees applying for a new card should indicate Corporate ID: 011000000109124 EDS employees applying for a new card should indicate Corporate ID: 011000000110349 • Complete the rest of the fields in full • Complete section 3, 4 & 5 where applicable. • Read the declaration at section 6, sign and date in “Signature of Applicant” box. Leave Signature of Authorised Signatory & Company Stamp fields blank. • Scan completed forms together with latest salary slip / complete signed offer letter (incase of new employee), and a copy of the NRIC (front & back) For Singaporeans and Permanent Residents / a copy of Passport or Employment pass in case of Foreigners. • CMs will need to ensure that the residential address provided on the card application form matches the address on the ID document (NRIC or passport).  If different, CMs will need to provide documents like Utility bills, bank statement, Credit Card statements issued within the last 3 months showing the residential address on the card application form. • and email to ajpcontactamex@hp.com for processing . • If any of the relevant fields are missing, DAR will return the application form to applicant for correction and resubmission. • Items to note: • Under MAS (Monetary Authority of Singapore) guidelines, Singapore card applicants need to have an annual income of at least SGD 30,000 before he/ she is eligible to apply for the corporate card. • Normal processing time for your Corporate Card Application is around 10 workings days upon receipt by American Express; missing or inaccurate information may lead to delays.

  2. Corporate Card Application Guide • If you need your card urgently, tick the box on the top portion of your application form & your application will be given priority handling. A fee of SGD $55 priority handling fee will be billed to your Corporate Card Account. Please allow turn-around time of 3 days (upon receipt of card application by American Express) for urgent card to be delivered. • Application status enquiries can be made to Amex Telephone Service Centre at 1800 732 2566 – Amex will only be able to advise status once the application has been received & processed.

New Card Application Process – Staff with > S$30K annual income iii) Staff forwards completed application form + latest income + ID document to Dedicated Account Resource (DAR on apjcontactamex@hp.com for employee verification i) Staff goes on Internet and download Card Application (LL) form ii) Staff completes, dates & sign in “Signature of Applicant” box v) DAR will forward approved application form to Amex Singapore New Accounts via email or fax for processing. iv) If any of the relevant fields are missing, DAR will return the application form to applicant for correction and resubmission.

Leave Signature of Authorised Signatory & Company Stamp fields blank. • Scan and submit completed application form, together with 1) Letter of Undertaking on the Company’s Letter Head and 2) Travel Itinerary 3) latest salary slip / complete signed offer letter (incase of new employee) 4) copy of the NRIC (front & back) For Singaporeans and Permanent Residents / a copy of Passport or Employment pass in case of Foreigners. to Dedicated Account Resource (DAR) via email to apjcontactamex@hp.com for processing. • Submit the undertaking letter and Travel Itinerary in Original to American Express office via Mail/Courier/In person ( Add :American Express, 20 (West) PasirPanjang Road, #08-00 Maple Tree Business City, Singapore 117439, Attn: New Accounts) • CMs will need to ensure that the residential address provided on the card application form matches the address on the ID document (NRIC or passport).  If different, CMs will need to provide documents like Utility bills, bank statement, Credit Card statements issued within the last 3 months showing the residential address on the card application form.

Temporary Card Application Process – Staff with < S$30K annual income ii) Staff completes, dates & sign in “Signature of Applicant” box Completed the undertaking letter and print it on the Company’s Letter Head iii) Staff emails completed scanned card application form, 1) Letter of Undertaking and 2) Travel Itinerary 3) latest income 4) ID document to Dedicated Account Resource (DAR) on apjcontactamex@hp.com and should submit the Undertaking letter and Travel Itinerary in Original to American Express Office through Mail/Courier/In person i) Staff goes on Internet and download Card Application (LL) form and The Letter of Undertaking v) DAR will email card application form, together with Supporting Documents 1 + 2 to Amex Singapore New Accounts for processing. iv) DAR will return any non-complete application form for correction and resubmission
