  2. Presentation Frame • Why aren’t we going over QA & documentation? • Purpose: • To experience and learn what other county’s case planning process looks like. • To get an outside perspective of your case planning process.

  3. Active Learning • Splitting into groups • Read the caregiver’s story • Case managers: You have 25 minutes to introduce the caregiver to your case management process and create a case plan with the caregiver.

  4. Caregiver Debrief • Caregivers, how was that for you? • What did the case managers say/do that felt good to you? • What worked? • What aspects of the case management process did you appreciate?

  5. Case Manager Debrief • Based on the feedback: • What would you suggest? • What might you do differently?
