  2. UNINSURED SERVICES Please be aware that some services offered at AHG are not covered by OHIP. These uninsured services require payment by the patient. The following is a list of some examples of such services:

  3. Insurance-related sick notes/forms 01 School/camp physicals 02 Truck driver’s physical 03 Sick notes, back-to-work notes, daycare notes 04 Attending physician statements 50 Immigration letters/invitations 06

  4. TRAVEL-RELATED SERVICES: Travel immunizations (per injection, not covered by OHIP). Please note that we do not offer travel counseling at our clinic. For comprehensive travel advice and recommendations, we highly recommend visiting a specialized travel clinic. • Travel cancellation insurance forms/certificates

  5. Please note that the TB test for educational purposes requires proof and is covered by OHIP. If you have any questions or concerns about these uninsured services or their associated costs, please do not hesitate to contact our office for more information.
