  1. Who provides the information in the PHR? Basics about Personal Health Records A personal Health Record (PHR) is a comprehensive collection of information about an individual’s health. Being created and maintained by the individual, it allows his or her physician or providers to enter and access relevant data. Personal Health Records (PHRs) differ from Electronic Health Records in it that the latter is maintained by the providers while the former is entirely owned and managed by the individuals themselves. A PHR is in fact made of several reports. Its content describes about the health conditions of the patient since his or her birth, describing the patient’s health and the related details in a complete and self-explanatory way.   2. By law, patients are entitled to have total control and access to their own PHRs and they can decide about who can have access to the same. There is even provision for fixing how much and which portion of a PHR is accessible by others. Normally, family members and relative are given access.
