How Would You Treat an Obese Smoker with a History of 4 Prior Failed Surgeries? A Case Report courtesy of Kent Ellington, MD OrthoCarolina Foot & Ankle Institute, Charolotte, NC Surgery #1: ORIF of talar body, excision of lateral talar process and medial malleolar osteotomy.y Surgery #2: (8 Months Post-ORIF) TTC fusion with lateral plate, iliac crest aspirate and allograft with stem cells. Surgery #3: Ankle arthrodesis with hardware removal, lateral closing wedge tibial osteotomy, and placement of a blade plate and autograft. Surgery #4: Ankle arthrodesis with hardware removal, placement of antibiotic beads and circular wire external fixator.
Surgery #5:Revision ankle arthrodesis performed using the DynaNail® TTC Fusion System. No supplemental graft materials nor biologics were used. 6 mm of available compression Immediate Post-Operative: The DynaNail’s Compressive Element is in its fully stretched position holding 6 mm of compression across joints. 1 Month Post-Surgery: Patient was placed in boot and allowed touch down weight-bearing. 4.2 mm of available compression 3 Month Post-Surgery: CT scan revealed bone consolidation. The Compressive Element was still holding 4.2 mm of compression indicating the joint had experienced 1.8 mm of resorption. 1 Year Post-Surgery: X-Ray scan shows solid bony union across the ankle joint. The patient experienced no complications and was happy with the results using the DynaNail. MedShape DynaNail.
