RESEARCH QUESTION http://news.mongabay.com/2008/0618-suriname_mining.html http://sranan-news.blogspot.com/2009/02/hoofdkapitein-anapaike-eist-dat.html Gold Mining = Mercury Pollution = Contaminated water = Diarrhea? U5MR?
SPECIFIC QUESTIONS Is water being used by indigenous people and maroons polluted by the mercury used in gold-mining? Is there an increase in the incidence of diarrhea in children < 5 in the period after the scale up of gold-mining compared to the period prior? Is there a difference between incidence of diarrhea in children < 5 in villages in gold-mining areas with incidence in matching villages in other areas? Is there an increase in the < 5 child mortality rate with diarrhea as the underlying cause?
STUDY POPULATION Small populations Geographically contained
DESIGN I. O1 O2 X O3 O4 a) Kwakoegron - series b) Bigi Poika - series II. O1 X O2 O3 O4 a)Kwakoegron & Makakriki - control b) Bigi Poika & Carolina - control X= scale up of gold mining
DATA COLLECTION HHS (questionnaire): females of childbearing age Routine data (checklists/compilation sheet) from civil services bureau (vital registry) Routine data (checklists/compilation sheet) from health care facilities: - admission/discharge (incl. transfer) - patient charts ** Direct Observation (QC)
INDICATORS # of cases of diarrhea reported by interviewees # of deaths related to diarrhea reported by interviewees # of children under 5 admitted to local Medical Mission post with diarrhea # of children under 5 admitted to Academisch Ziekenhuis with diarrhea # of deaths in children under 5 as a result of diarrhea (either while at MM or at AZ)
PRESENTING THE DATA Note: Gold Bar represents gold mining activity scale up Blue Bar represents “flood of 2006”
LIMITATIONS/ISSUES • Confounding variables: other risk factors for diarrhea • Other advershe health effects of mercury not researched
LIMITATIONS/ISSUES(continued) • Of all villages only Kwakoegron has actually requested assistance with this issue. • Fictitious data used due to time limitations and outdated info. • For “actual research” more lead time needed to identify appropriate villages as target populations.
DISSEMINATION/POLICY IMPACT • Research findings to be shared with stakeholders. • Depending on outcome village leaders may have more evidence to request interventions to address unsafe goldmining practices/contamination of water.
DISSEMINATION/POLICY IMPACT(continued) • Possible interventions to be discussed with village leaders --potential for follow up research to assess whether interventions actually work and health of indigenous and maroon children improves.
