Background • The program was authorized under the Public Health Service Act, Title II, Section 330A(g) in 2002 • ORHP delayed the start of this program to incorporate recommendations from the rural IOM report, “Quality Through Collaboration: The Future of Rural Health”, released in November 2004. • The two-year program began in fiscal year 2006; 15 awards were made (up to $50,000/year). • The program was re-competed in fiscal year 2008; 55 new awards have been made (up to $75,000/year).

  3. Purpose • The purpose of the Rural Quality Grant Program is to improve patient care and chronic disease outcomes for diabetes mellitus (DM) and cardiovascular disease (CVD) by assisting rural primary care providers with the implementation of quality improvement strategies. • The program focuses on DM in year one and CVD in year two. • Improvements are achieved by: • Use of an electronic patient registry system to track and report health indicators • Conducting organizational needs assessments to identify areas of improvement and implementing additional quality improvement activities • Participating in monthly technical assistance calls and 4 peer-learning workshops with fellow grantees

  4. Rural Quality Grantees • Eligible entities: • Rural health clinics (for-profit and not-for-profit) • Critical access hospitals (for-profit and not-for-profit) • Other rural health care providers • Grants • Awarded 55 Grants • Total Funding • FY 08 $4.03 million • FY 09 $3.99 million

  5. Grantee Focus Areas • Implement an electronic patient registry system • Use the Chronic Care Model for Improvement Plan (i.e., Plan- Do- Study-Act) • Self-management and patient-centered care • Measure and track performance with health indicator data Six Rural Quality Health Indicators
