  2. Radiation Safety Work Control Form • RSWCF is used to manage work on configuration controlled radiation safety systems at SLAC: • Personnel Protection System (PPS) • Beam Containment System (BCS) • Shielding

  3. Radiation Safety Work Control Form • Each SLAC organizational unit responsible for operating an accelerator facility has a RSWCF process RSWCF format and minor details vary for each facility, but the process is the same across the lab.

  4. Radiation Safety Work Control Form • Step 1: Identify work that requires a RSWCF. • Labeling • Training • CATER* integration • ADSO and Area Manager of new CATER entries * CATER = Computer Assisted Trouble Entry and Reporting

Radiation Safety Work Control Form • Step 2: Designate “Person Responsible,” initiate a RSWCF, describe radiation safety implications of work, and get Area Manager approval to proceed with work planning. • Person Responsible: • Directly doing or overseeing work • Understands: • Scope of work • Radiation safety implications • RSWCF process • Responsible for only allowing work when RSWCF is in the “Work Approved” state.

  6. Radiation Safety Work Control Form • Step 3: ADSO completes “Requirements Before Starting Work” and “Work Completion Requirements. ADSO and RP sign. Person Responsible Signs.

  7. Radiation Safety Work Control Form • Step 4: Operator In Charge (OIC) ensures that requirements before starting work are met, then signs to move the RSWCF to “Work Approved.” • Hold a PPS key in a keysafe • Padlocks in place locking off hazard • Stoppers disabled Step 5: Do the work!

  8. Radiation Safety Work Control Form • Step 6: Person Responsible or Area Manager complete work completion fields.

  9. Radiation Safety Work Control Form • Step 7: ADSO reviews work completion fields and moves form to “Work Complete” state. Step 8: OIC reviews and signs to move RSWCF to the “Closed” state. Hazard controls may now be revoked.

  10. Radiation Safety Work Control Form • Linear Accelerator Facility electronic RSWCF rolled out in January 2011 • ~ 1450 forms in 7 years • SPEAR-3 Accelerator Facility started using electronic RSWCF in May 2012 • ~700 forms in 6 years • Both Facilities • 2 to 10 per maintenance day • ~ 30 RSWCFs per extended downtime
