LTPAC and LCP Use Case Suggested Roles • LTPAC takes lead on: • LTPAC transitions aspect of Use Case • Ensuring actionable scope for implementation • LCP takes lead on: • Home health Use Case integration • Articulating long-term vision for the Longitudinal Care Plan • LCC WG (all members): • Review, comment and consensus on Use Case
