Our Mission and Aims To establish a comprehensive method of real-time data collection which is embedded into the stroke patient’s journey to enable accurate reporting and therefore improve measured performance and enhance patient care.
Case for Change • Currently required to report retrospectively on IPMR, AQ, and SSNAP as well as local KPIs • Data collection & entry time consuming and detracts from clinical care • Single real-time data collection solution required for data collection at the point of care • Options: Spreadsheet, Kaizen, Cerner, commercial product .

Our Approach Staff conversation –26/04/13 combined with planned Stroke Coordinators training event, including Stroke Doctors, Data Co-ordinator, Ward Manager SEG Data Group meetings to engage MDT
