Logic Model for Kansas UMC Healthy Congregations in Action Inputs Strategies Outputs Outcomes Evaluation Healthy People 2010 Outcomes, Insurance Usage Data, Needs Assessment & Survey Data • • Create & adopt a communications plan to support development & implementation of the Healthy Congregations in Action initiative • • Identify & develop leadership for initiative • Provide multi-faceted health education/skill development programs & activities • • Model & promote healthy lifestyles • Create environments that encourage health & well-being • Develop & implement a multi-level evaluation plan to assess processes & outcomes • “Safe Places” for clergy to access personal health support resources • Place-based tailored programs & interventions • Church-specific health behavior data • Partnerships for health outcomes • Training & resources for personal health covenants Training & resources for health leadership development Environmental & policy change resources relative to health outcomes • Short Term Outcomes: • • Greater acceptance by laity of church’s role in health • • Increased acceptance & support of the practice of healthy lifestyles • • Increased knowledge, attitudes & skills relative to healthy lifestyles • • Increased availability of health programs & activities in response to identified needs • • Increased awareness & acceptance of confidential health support resources for clergy, congregations & conference leadership • • Increased commitment within the church regarding health • • Increased numbers of clergy & laity with personal health covenants • • Clergy & laity become advocates & role models for healthy lifestyles • Intermediate Outcomes: • Lifestyle activities that support physical, mental/emotional, social and spiritual health are integrated into the life of the church • Increased wellness ministry leadership roles exist for clergy, staff & laity • • Increased practice of healthy lifestyle behaviors in all four facets of health: physical, mental/emotions, social & spiritual Kansas Area United Methodist Healthy Lifestyles Committee Community Capacity • Shared faith and traditions • Human Resources: core expertise, diversity, sensitivity • Facilities • Communication systems • Administrative capacity Relationships with Partners •United Methodist Health Ministry Fund • Community Partners • Medical School Partners • Medical Community Partners • Business Partners Contextual Conditions ( Differences in size, ethnic makeup, socio-economic conditions, resources, experience, and clergy support) Long-Term Outcome Healthier clergy, congregants & community partners
