GOALS & SERVICES • Reduction of High Profile Underground Cable Damages • Reduction in Overall Damages • Reduction of Contract Locator Damages • Education of Contractors on Safe Digging Practices • Minimize Underground Cable Repair Costs • Assist AT&T Risk Management Team in collections from contractors. • Provide real live interface on excavations • Provide AT&T status on X-BOX/Plant conditions (damaged doors, Graffiti, etc.) • Physically perform field inspections on USIC (locating contractor)

  4. DAMAGE PREVENTION INSPECTOR JOB RESPONSIBILITIES • Each Inspector has an assigned coverage area. We currently have a total of 12 DPIs throughout South Florida. One of those inspectors is responsible for Inspections of our locating contractor’s accuracy of locates. • The remaining inspectors, review all HP tickets that were located on previous day, review list of permits issued in their counties and contact those contractors. Our DPIs are available to stand by during excavations near high profile facilities on road jobs. • Contact is made with all contractors with HP tickets to find out when work is taking place. • Each job site is visited to educate contractors on safe digging practices; to make sure soft digs are done when crossing our duct runs or when bores are being conducted, to ascertain that excavation with machinery takes place only outside tolerance zone and prints/plats are reviewed to make sure all cables are accounted for in the field. • Throughout the day and during their travels, the DPIs will stop whenever an excavation is observed to verify if the excavator has a valid locate ticket. If unable to provide documentation that locates were requested, and if there are no locations in place, the contractor will be asked to stop their work, request locates and wait two full business days for the marks to be completed. If excavator refuses to stop digging, Code Enforcement is notified.

  9. Permit issuance report • Damage Prevention Inspectors visit sites where permits have been pulled for excavation and meet with contractors BEFORE excavation begins to take place. This preemptive action has helped reduce damages by educating the community on how to call in Sun Shine One Call location tickets. • Fact: Many counties do not educate their contractors and homeowners on safe excavation practices to include requesting underground locates from SSOCOF. • National Services together with AT&T and SSOCOF are working together to promote the education of Code Enforcement and Police officers on implementing State Statue 556. • *Pilot program is currently going on in the city of Bal Harbour .

  14. Damage Investigations by national services • Damage Prevention Inspectors document reports of any high profile underground utility damages to be provided to AT&T’s Risk Management Department. This has resulted in expedient collection of claims. • DPIs are required to respond to high profile damage sites as soon as the damage is reported in order to help determine liability. • Each inspection is to include photograph of the damage and the excavator at the site to provide evidence to be used by Risk Management. • National Services is the first on site to help our master contractor determine who was the party responsible for the damage; helping AT&T collect payment of the damaged facility much faster!

Dpi disaster restoration team • PROTECT: • ALL NEW AERIAL CABLES RESTORED TO AREA, • WATCH HEIGHT ON DEBRIS REMOVAL TRUCKS, AS NOT TO INTERFERE WITH AERIAL DROPS AND CABLES. • WATCH EXCAVATORS REMOVING AND LIFTING DEBRIS, SO THAT THE PEDESTALS DO NOT GET PULLED OUT OF THE GROUND • KEEP TRACK OF EACH COMPANY REMOVING DEBRIS BY PLACARD NUMBER • MANAGER TO GIVE SAFETY MEETING TO HAULERS AT THEIR SAFETY MEETING, AT LEAST ONCE.

Dpi disaster restoration team • ENFORCEMENT: • CHECK AREA 811 LAWS • CHECK FOR VARIANCE LAWS • EXCAVATION LAW • CHECK FOR VALID DIG TICKETS ON ANYONE DIGGING, IF NO TICKET IS FOUND, TO PROMOTE AND MAKE AWARE,THE 811 SYSTEM, AND TO ASK THE HOMEOWNER, CONTRACTOR, ETC, TO PLEASE OBTAIN TICKET ON SITE AND WAIT THE ALLOWED TIMEFRAME FOR THE AREA TO BE LOCATED, SO THAT THEY MAY DIG SAFELY AND TO KNOW WHAT IS BELOW. • TO STRESS THE POINT THAT WE ARE HERE TO PROMOTE SAFETY, AND THAT WE ARE NOT HERE TO STOP THE PROGRESS OF RESTORATION, WE WILL MAKE THE CALL FOR THEM IF THEY HAVE NO ACCESS TO PHONE SERVICE. • EMERGENCY TICKETS CAN BE PULLED, GIVEN THE RIGHT CIRCUMSTANCE. • WE WORK WITH THE COMMUNITY, CONTRACTORS AND LAW ENFORCEMENT TO HELP RESTORE THE COMMUNITY BACK TO HOLE AGAIN.
