  4. 消化道的基本組織排列（由內到外） • 黏膜層（mucosa） • 黏膜下層（submucosa） • 肌肉層（muscuaris） • 外膜層（serosa）

  5. 附屬消化器官 • 牙齒 • 舌頭 • 肝臟 • 膽囊 • 胰臟

  6. Pharynx

  7. Cavities communicating with the pharynx • Nasal cavities 和tympanic cavities ↔ Nasopharynx • Mouth ↔ oropharynx • Larynx 和esophagus ↔ laryngopharynx

  8. Esophagus • 大約25cm長，由喉咽延伸到胃 • 開始於環狀軟骨（C6），終止於胃（T11） • 胸主動脈的下降枝（descending thoracicaorta）是介於食道與胸椎之間

  9. 食道的三個狹窄處 • Aortic arch • Esophagus crosses the left bronchus • 近heart處

  10. Esophagram • 主要評估pharynx和esophagus功能 • 利用放射線與contrast medium或 barium使用做為診斷 • 評估吞嚥功能正常與否

  11. Indication • anatomical anomalies解剖上的異常 • Foreign body obstruction • Esophageal reflux • Esophageal vances靜脈曲張 （大多與acute liver disease有關）

  12. Contraindications • 年紀太大無法吞服顯影劑 • 懷疑有食道破裂時則考慮使用 water-soluble contrast medium

  13. Barium 濃度 • 通常一包鋇劑為600g • Esophagram，UGI濃度為1：4 （600 ㏄的水泡4包） • LGI檢查濃度1：3 • Single Ex 1000 ㏄水泡350g鋇劑 • Double Ex 600 ㏄水泡3包

  14. Patient preparation • 原則上NPO 4小時 • 去除身上的異物→更換病患服 • 若有需要評估UGI series的部份，則 按照UGI的precedence

  15. Routine position for esophagram • RAO • Left lateral → foreign body • AP view

  16. Cardiac series • PA • Left lateral → 評估heart是否肥大 • 450 RAO • 600 LAO • 亦可使用其他檢查來評估heart， ex：echocardiography or ultrasound 因為超音波可直接作測量功能，評估 心臟瓣膜是否異常

  24. Single enlarged fold that arises at the gastric cardia and extends upward into the distal esophagus. “Inflammatory 發炎esophagogastric polyp”

  25. Smooth tapered segment of concentric narrowing Hiatal hernia

  26. Fulminant candida esophagitis = AIDS+ candida esophagitis Shaggy (毛茸茸) esophagus Innumerable pseudomembranes and plaques

  27. Multiple small superficial ulcers surrounded by radiolucent mounds of edema

  28. Two superficial ulcers

  29. Long smooth stricture caused by lye ingestion
