  2. Steps to take when injured. • Notify your supervisor immediately, even if you think the injury is minor. Your supervisor will contact the Workers’ Comp/Facilities Department. • Complete an Employee’s Notice of Injury. This should be done within 24 hours of the incident. • If seeking medical, you must see a worker’s comp doctor. Contact the Workers’ Comp/Facilities Department at x4075 to let them know you are seeking medical and to discuss your options. • An Accident Investigation Reportwill be completed by the Principal/Director.

Workers’ Comp Fraud • Fraud costs the Texas workers’ compensation system millions of dollars each year. • Fraud occurs when a person knowingly or intentionally conceals, misrepresents, and makes a false statement to either deny or obtain workers’ comp benefits or insurance coverage, or otherwise profit from the deceit. • Please report any fraudulent activity to x 4075.
