Health InsuranceWellmark Blue Cross Blue Shield • Eligibility Requirements: • 20 or more hours a week with an appointment for 9 months or longer • Date Coverage Begins: • First day of the month following the date of employment • Pre-Existing Conditions: • No waiting periods • Submit health/dental enrollment form within 30 days after your date of employment
UNI PPO Insurance Plan • Preferred Provider Organization (Alliance Select PPO) • Selecting an in-network health care provider may mean you will pay less for out-of-pocket expenses • In Iowa: • 100% of the hospitals participate in Alliance Select • 99% of the providers participate in Alliance Select • View Coverage Manual

UNI Blue Advantage Network • Emergency Care When Traveling • No coverage outside of the Iowa/South Dakota network, unless: • Accidental Injury • Emergency • Case Management Referral
Primary Care Providers (PCP) • Designate an in-network PCP when you enroll • PCP’s Include: • General & family practitioners • Doctors of internal medicine (internists) • Pediatricians • Advanced Registered Nurse Practitioners (ARNP) • Physician’s Assistants
Primary Care Providers (PCP) • Females may also choose a secondary in-network PCP (OB/GYN) to coordinate care • Routine physical must be completed by a PCP or OB/GYN • PCP’s can be changed at any time. • Effective date of change is the first of the next month • Contact Wellmark customer service to update your PCP
Guest Membership • Outside the Wellmark Blue Advantage network for at least 90 days: • Traveling long-term • Covered dependents attending college out-of-state • Covered family members living apart • Not all services are covered • Must designate a PCP for out-of area location
Mail Order Prescription Drugs • Employees enrolled in UNI PPO or UNI Blue Advantage are eligible • Receive three months of a prescription for two monthly co-payments • Convenient and safe prescription mail order service through Immediate Pharmaceutical Services

UNI Dental Plan 2 Coverage • Preventive Maintenance Care – Delta Dental pays 100% • Routine Restorative Care and Surgical Care – Delta Dental pays 80% • Major Restorative Care and Dental Prostheses – Delta Dental pays 50% • Maximum Annual Benefit – $1,500 per person per calendar year for all services combined • Orthodontic Care – 50% up to a $1,500 lifetime maximum per member • View Coverage Manual

Pre-Tax Premium Plan • Allows employees to pay family health and/or dental insurance premiums with pre-tax dollars • Premium is deducted before federal and state income tax, and FICA taxes are calculated • Participation limits the changes that can be made to plans during the plan year
Adding/Dropping Dependents • Life Events • Birth, adoption, or placement for adoption • Marriage • Spouse loses and/or gains coverage • Death • Divorce • To change your benefits, you must notify UNI within 31 days of the life event • In the case of birth, adoption, or placement for adoption, you must notify UNI within 60 days of the event.
Open Enrollment Periods • Changes and new enrollments are effective on August 1st for: • Health • Dental • Changes and new enrollments are effective on January 1st for: • Vision • Flexible Spending
Avesis Vision Plan • Nationwide network of providers • Pre-tax payroll deduction • Covers materials only • Includes glasses or contacts • Reminder: an eye exam is covered under your health insurance plan • Monthly Premium: • Employee Only - $8.15/month • Employee + Family - $18.76/month
EyeMed Vision Discount • If you are enrolled in Delta Dental of Iowa, you are eligible for a vision discount program through EyeMed Vision Care at no extra cost • Visit Delta Dental for more details on the discounts offered and a list of participating providers • You’re automatically enrolled • This discount program does not work in conjunction with Avesis; you must use one or the other
Life Insurance • Eligibility: • 20 or more hours a week with an appointment for 9 months or longer • Date Coverage Begins: • Automatic enrollment on the date eligibility requirements are met • Cost: • Full premium is paid by UNI • View Coverage Manual
Life Insurance • Coverage is 1.5 times the employee’s annual budgeted salary up to a maximum benefit of $250,000 • Life insurance benefit reduces by 35% at age 65 • Beneficiary: • Complete a Beneficiary Designation Form and update your beneficiary as necessary
Voluntary Term Life Insurance and AD&D • Voluntary term life insurance (VTL) and accidental death & dismemberment (AD&D) coverage is available to employees, spouses, and children • Through Principal Life Insurance Company • Monthly payroll deduction withheld after-tax • View Coverage Manual
AD&D • AD&D coverage is automatically included for those covered under the voluntary term life insurance • Benefits will equal the voluntary life insurance coverage elected
Voluntary Life – Employee • Employee Coverage Amounts: • Minimum amount: $10,000 • Maximum amount: $300,000 • Purchase benefits in $10,000 increments • Guaranteed coverage under age 70 is $300,000 • Guarantee coverage age 70 and over is $10,000
Voluntary Life - Spouse • Employee coverage is required for spouse to elect coverage • Spouse Coverage Amounts: • Minimum amount: $5,000 • Maximum amount: $50,000 • Purchase benefits in $5,000 increments • Guaranteed coverage under age 70 is $50,000 • Guarantee coverage age 70 and over is $10,000
Voluntary Life - Children • Child Coverage Amount • For eligible children age 14 days or older: • $10,000 of coverage – number of children covered is unlimited • $2.00 per family/month • For eligible children under 14 days of age: • Employees who elect the coverage receive $1,000 of coverage
Voluntary Life – Portability • If an employee leaves UNI, they can continue their voluntary term life coverage • Rates will remain the same as the UNI group rates for at least 12 months
LTD Insurance • Eligibility: • 20 or more hours a week with an appointment for 9 months or longer • Date Coverage Begins: • Automatic coverage after one year of service • Cost: • Full cost is paid by UNI
LTD Insurance • Provides salary replacement if an employee is totally disabled • Benefits begin the later of 90 working days or expiration of all accrued sick leave
Supplemental LTD Coverage • Allows eligible employees to purchase 70% LTD coverage prior to their normal eligibility for University paid coverage • Employee pays the full premium through payroll deduction
LTD Insurance Retirement Protection Plan • Continues retirement contributions and life insurance for employees who qualify for disability benefits and are enrolled in TIAA-CREF • Coverage is automatic and the premium is paid in full by UNI
