Roles & Responsibilities Management of SMS implementation plan Performs hazard ID & Analysis Mitigates with corrective actions Monitors system for compliance to standard Collects and analyses all data-trends Maintains documentation and records Provides safety training Holds regular safety meetings

  4. SAFETY COMMITTEE RESPONSIBILITIES Monitors effectiveness of SMS Monitors corrective action is timely Monitors safety performance against objectives Monitors effectiveness and safety of contractors Ensures appropriate resources are allocated Provides lessons learned to all personnel Oversees SMS compliance with standard

  5. Safety Officer daily responsibilities Cursory inspection of facility/AC Review of compliance calendar Schedule and conduct required training Ensure safety docs up to date/court ready

  6. AUDIT RESPONSIBILITY Safety Officer shall perform internal audits on established schedule Safety Officer may delegate audit Safety Officer must ensure auditor is trained

  7. ACCIDENT INVESTIGATIONS The Safety Officer is responsible to investigate all incidents and accidents to determine root causes. Root cause analysis is a formal written process The Safety Officer will implement mitigations and additional strategies to prevent a reoccurrence of accident The Safety Officer will ensure all personnel are advised and trained regarding new procedures

DOCUMENTATION • The Safety Officer is responsible to establish SMS filing system and to keep it secure and controlled. • Files are kept for the following: • JHA’S • WI’S • OPEN C.A.R.’s • CLOSED C.A.R.’s • TRAINING RECORDS • ACCIDENT REPORTS • CORRECTIVE ACTION REPORTS • ASAP REPORTS • INVESTIGATION REPORTS • DATA COLLECTION

TRAINING REQUIREMENTS • Safety Officer must schedule/conduct all safety trg. • Safety Officer must provide basic SMS fam. training to all personnel to include: • Hazard communication • Tug/equipment certifications • Lockout-tag out procedures • Confined space safety, PPE, electrical safety • Hazardous materials • ERP-emergency evacuation procedures • Use of all safety equipment

Other Responsibilities Maintain court ready documentation Lead safety committee Maintain contractor and certificates of insurance Obtain and maintain permits on any equipment Search for avenues of continuous improvement Lower accident/incidents rate Interface with regulatory agencies Develop and implement visitor safety program Provide top management with ideas
