    2. Using the list of approved providers, send an e-mail asking if they are willing to come to your district. Include: Where you are located Number of SES slots you have available A deadline to respond. 1. Found out which providers are willing to serve your district.

    3. The procedure must be easy for parents to follow. Create an enrollment form: Include a list of all available SES providers. 2. Develop a procedure for enrolling in SES.

    4. Parent friendly language – “Free Tutoring,” Information about the providers, Procedure for signing-up Explanation of the benefits of SES A statement that indicates there are limited slots available. Enrollment form (optional) 3. Send a letter to the parents of all students that qualify for F/R Lunch in the schools required to offer SES, including:

    5. Agreement for providing services Student Learning Plan With the provider and parent, set a growth goal based pre- and post-testing. The SLP is the responsibility of the LEA. 4.Develop contract(s)

    6. Number of students who were eligible for and the number of students who participated in supplemental educational services (beginning with 2007-08 school year and for each following year). For the current school year, a list of supplemental educational services providers approved by the State willing to serve in the district and the locations where services are provided. 5. Post on the district website:

    7. Observation Checklist Observe one tutoring session per semester for each SES provider. Performance Report Complete one per semester for each SES provider. Evaluation Report Following the instructions, submit one report by June 1. 6. Monitor and Evaluate

If the LEA wishes to use the remaining funds for other allowable expenses, the LEA MUST: Allow SES providers access to schools, Distribute information directly to qualified students and broadly disseminate information, Set at least two enrollment windows or allow continuous enrollment, Partner with outside organizations, Notify the SDE. 20% Obligation
