Washington Street Cost: $8 per child. Includes admission to the Science Center We will be attending a performance at the planetarium regarding the planets at 9:30. Then, we will take some time to walk through the Science Center. __________________________________________________ My child, __________________, can attend the Science Center field trip. ____________________________ Parent Signature Every child must travel in a car with adequate seat belts. If you are interested in driving, please fill out information below. Chaperones will have to pay $8 as well as parking cost. ________________________ ________ Name of Driver # of seats
