Target Patient Group Bury- 65-74yrs All others- over 75yrs Routine Elderly check or contact- Enquire about falls in the previous year and prior fragility fracture Is the stage 1 Falls & Fracture screen positive? All other positive returned Stage 1 Falls & Fracture Screens e.g. from voluntary sector, ambulance service, customer first Patient returned a Positive Stage 1 Falls & Fracture Screen questionnaire WS QP11A Pathway Process Patient added to Falls Register Yes No Health & Well-Being Advice Register Stratification Number of Patients under the care of Community Provider Local Healthcare Teams Number of patients living in a 24hr Care setting Number of housebound patients (Not currently Under the care of LHcT) Number of Parkinson's Disease Patients under the care of Parkinson's Disease Specialist Nurse Number of patients with diagnosed dementia known to MATS Number of patients who have already had a falls & fracture assessment checklist completed All Others Return the numbers on the audit template to sandie.robinson@suffolkpct.nhs.uk by 30th June 2012 Enquiries to Ann Hunt or Nicola Craig ann.hunt@suffolkpct.nhs.uknicola.craig@suffolkpct.nhs.uk Version 2 QP11A
