  2. Start with culture change • A focus on outcomes • Participation of children, parents and young people in decision-making • Joint working across agencies, services and institutions

  3. From September 2014 All new assessments will be Education, Health and Care assessments and initial local offer should be published. Transfer of statements and to EHC plans begins, but can take up to three years. New Code of Practice comes into force.

  4. The Local offer is to: • Build on existing legal requirements (e.g. short breaks statement) • Review what education institutions provide from their delegated budget • Provide information on local services.

  5. Transfer to EHC plans • Transition period extends to April 2018 • A local strategy co-produced with children parents and young people • Prioritising the transfer of children and young people at key transition points

  6. Early conversations with children, young people and parents Person centred and focus on outcomes - avoiding a ‘change of name’ approach Full re-assessment may not necessary - existing evidence can be used if it is recent and relevant

  7. School stages • Early discussions with parents and children on the transition • Review how parents and children are engaged in the SEN process • Use the SEND Code of Practice to plan
