EIRA1 completion recommended Yes PI to consider ethical implications then required to complete EIRA1 (ethics form) and pass to second reader for review If straightforward: Second reader and HOD sign off EIRA1 If more complex: PI to discuss with DREO and 2nd reader and take action prior to sign off by HOD If potentially significant issues: refer to appropriate Dept/Faculty/School committee If there is no appropriate body, then refer to University Ethics Committee. PI to take action. PI to give EIRA1 back to DREO for incorporation in Annual Department Monitoring Report University Ethics Committee receives: Department Annual Monitoring Reports (EIRA2), via Faculty Research Committees (and identifies and disseminates examples of good practice via website and letters to HoDs/DREOs for planning future training/workshops) Faculty Research Committee minutes Review of ethical issues referred from Departments or Faculties Senate informed of routine issues via minutes of the University Ethics Committee Any exceptional issues would be dealt with in a separate report Note: EIRA1 = the University Ethics form
