What is SOS & why do we need it? • UCI program that meets Cal/OSHA’s Injury & Illness Prevention Standard • Integrates Safety into our Workplace • Injury Minimization • Workers’ Compensation Cost Reduction • Creation of a Safety Culture

SOSRepresentative Responsibilities • Communicate SOS program elements to work unit members • Ensure work unit members have taken the self-assessment in UC Learning Center and EH&S required training • Complete the Hazard Assessment and Corrections Tool • Complete work unit specific training • Communicate completion of steps and safety related activities to EH&S
How can my work unit meet the requirements? • Ensure appropriate training and awareness of hazards • Ongoing participation in safety activities • Encourage individual accountability for safety
UC Learning Center • UC Learning Center • Ensure that work unit members log into UC Learning Center and complete the Self-Assessment • Questions evaluate job categories/activities • Responses determine required & recommended training • Supervisors/PI’s have access to transcripts • Update whenever new hazards arise
Required Training • Ensure that new employees complete required training in UC Learning Center: • Core/Lab Core Safety within one month of hire • All other EH&S training within 3 months, if reasonable • Participate in work unit specific training • Remain current on all training
Hazard Assessment & Corrections Tool • Identify Hazards • Consider daily tasks and activities that your work unit must perform • Consider tools, materials, and equipment • Correct Hazards • Develop Standard Operating Procedures • Include in work unit specific training and refer staff to EH&S training • Documents work unit specific training
How do I use this tool? • Identify Hazards (column 1) • Identify Follow-up Actions and Corrections or Controls (column 2) Safety Representative:• Assess each item below by walking through work unit and/or surveying employees. • Check all that apply. • Add additional information, if applicable. Identify follow-up actions, if necessary: • Fix problem• Refer staff to EH&S training• Create workgroup to solve issue• Develop work unit specific Standard Operating Procedures (SOP)• Include in work unit specific training
How do I use this tool? • Document and Date Completed Corrections (column 3) Enter date follow-up action is complete.

Frequency: Hazard Assessment & Corrections Tool • Complete tool every three years • Complete earlier if: • Significant change to environment • Significant change to operation • New employees • New hazards • Maintain SOS Records Indefinitely

Lookout for Changes • Ensure that when new hazards arise and when employees change job duties they: • Retake self-assessment • Enroll in additional required EH&S classes • Update Hazard Assessment and Corrections Tool • Receive work unit specific training • Ensure that employees who change supervisors: • Update supervisor’s name (My Desktop>Update Profile)
Rewards Level 1: SOS Representative has completed safety self-assessment and has taken SOS Representative training $5 EH&S Safety Dollars Level 2: SOS Representative has completed all hazard assessments and corrective action items under the work unit’s control $25 EH&S Safety Dollars Level 3: Work unit members have taken EH&S required & work unit specific training and are current on all re-training requirements All work unit members receive $5 Starbucks gift cards and choice of SOS items
Individual Participation Activities • Eligible safety participation activities • For each 2 activities completed, each member of the work unit will receive 1 entry in an annual raffle to win an iPod or similar device • Raffle will be held each January and SOS Incentive Level 3 must be completed
