Components of Successful Improvement Strategies • Identify goal • Establish a method to measure progress • Implement change • Evaluate change
Identify the Goal • Written statement • Be specific • Use numeric values • Be realistic but still make it a stretch
Identify the Goal Examples of Goal Statements*: • Identify 80% of all diabetics who have not had their HbA1c tested • Successfully complete 90 percent of all attempts to contact members who are due for a postpartum visit Source: Center for Health Care Strategies, Inc. (CHCS)
Establish a Method to Measure Progress • Should directly measure the written goal • Can be rapid-cycle with small samples • Should be measured over time across a wide range of variables/conditions • Should differ from the HEDIS measure you are trying to improve
Establish a Method to Measure Progress Example of Measurement of Improvement: Numerator: • Number of successful outreach attempts to members who are due for a postpartum visit Denominator: • Total number of members who are due for a postpartum visit
Implement Change • Identify who, what, where, and when • Sufficient training and communication are essential • Pilot test the change using a smaller group to readjust plans as needed. Consider multiple pilot tests to evaluate adjustments made.
Discussion of Case Studies • Recognize that not every intervention will fit your population, but consider the methods/approaches • Pay attention to “lessons learned” • Use the resources that are already available – do not reinvent the wheel
