  3. Name, Address etc. • Write name and address in capital letters • Write Mobile number (Ex. 09412903929) • Write Telephone Number (Ex. 01332266328) • Write your email is in small letters (Ex. xyz@gmail.com)

  4. Membership, Scheme and Branch Code • Write membership grade (ST/T etc.) • Write membership number (Ex. 584451-2) • Write Scheme code (Ex. 2 for Non Diploma stream, 3 for Diploma stream and 4 for Section B) • Write branch code (See next slide)

  5. Branch Code • Write branch code as 05 for Civil Engg., 11 for Mechanical Engg. Etc. • Architectural Engineering (AR) 03 • Chemical Engineering (CH) 04 • Civil Engineering (CV) 05 • Computer Science & Engineering (CP) 06 • Electrical Engineering (EL) 07 • Electronics And Communication Engg. (EC) 08 • Mechanical Engineering (MC) 11 • Materials & Metallurgical Engg. (MM) 12 • Mining Engineering (MN) 13 • Production Engineering (PR) 14 • Textile Engineering (TX) 15

  6. Previous Exam Detail Write roll number of previous exam (if you appeared). If you can’t remember, skip this field.

  7. Centre Code Write three digit Centre Code Example: 247 for Roorkee centre. 110 for Delhi centre. Etc. (for exam centre codes, visit www.amiestudycircle.com) You can opt for any exam centre. However, if you change your exam centre, pl enclose a reason with exam form.

  8. Write Subject Codes Write subject codes. Example: For Material Science in Section A, subject code is AD 302. (for subject codes, visit www.amiestudycircle.com) You can opt maximum four subjects.

  9. Section B Details • Write roll number of exam in which you passed Section A. • Exemption Details (If you got exempted in any subject(s) due to your qualification or any other reason)

  10. Signature Full signature is must. Without it, your form will be rejected.

  11. Demand Draft Details • Write • Amount • No. Of Demand Draft • Date of Demand Draft • Drawn on: Name of Bank

  12. Keep Xerox Keep Xerox of the form This is very important
