  2. Unique Features • Cover for complications arising out of Cardiac Surgery (CABG/PTCA) already undergone. • Also covers regular hospitalisation expenses for all other ailments

Coverage Section 1 : Hospitalisation Expenses incurred as an in patient for Accidental Injuries/Illness/Sickness/Diseases/(other than the complications arising out of cardiac surgery (PTCA/CABG) already undergone ). Section 2 : Hospitalisation Expenses incurred as an inpatient for any Cardiac related Complications

  4. Features • Insured person should have undergone for the first time PTCA/CABG between 6 months and 3 years prior to policy commencement date. In other words at the time of taking this policy the period between the surgery and policy commencement date should not be less than six months and not more than three years • Coverage offered under 2 Plans namely • Silver Plan • Gold Plan

Underwriting Guidelines • Pre acceptance medical screening for all persons irrespective of the age • Centralized underwriting at Corporate Office • Separate Proposal Form • List of documents to be submitted by the insured • Discharge Summary of PTCA/CABG already undergone along with the CD given at the time of discharge • Echo Cardiogram not older than 3 months • All investigations done before admission, during hospitalisation and during follow up.

  10. Underwriting Guidelines • Pre-acceptance medical at Company Nominated Centre at Company’s Cost • General Physical Examination • Fasting Blood Sugar • Serum Creatinine • ECG • USG Whole Abdomen • Urine Routine. • Echo Report, if the report submitted by the insured is older than 3 months.

Important Exclusions • Applicable Section 1 • Pre Existing Diseases up to 48 months of inception of the cover (except for cardiac ailments) • Any Disease contracted during first 30 days • Naturopathy Treatment • Stem cell implantation and / or therapy • Expenses incurred on Enhanced External Counter Pulsation therapy and related therapies and Rotational Field Quantum Magnetic Resonance Therapy and such other similar therapies • No First Year Exclusions • First Two Year Exclusions – For complete list, please refer prospectus/policy clause.

Important Exclusions(Contd…) • 10% of each and every claim amount for insured persons beyond 60 years of age at entry level and renewals thereafter. • Expenses incurred for treatment of diseases/illness/accidental injuries by systems of medicines other than Allopathic • Section 2 (Applicable for Section 2 only) • Expenses incurred for treatment in respect of all cardiac related complications during the first 90 days of coverage under the policy
