Objectives • Describe what is an outcome • Understand the context of outcomes in supported housing • Practice outcomes-focused support planning • State how support planning relates to the Supporting People outcomes
Outcome definitions • ‘End result’ • ‘Something that follows an action’ • ‘Final consequence’ • ‘A conclusion reached through a process’
Support Planning Process • 5 stages: • Make an initial assessment of need • Develop a support plan - SMART • Put the plan into action • Review of progress and reassessment of needs within a specified time • Set new goals and tasks
SP Outcomes Framework • 11 Outcomes areas • Questionnaire to complete for • all service users - mandatory from April 2012 • Data collected by WAG
Organisational responsibilities • Support Planning Policy & Procedure • Training • Case review meetings and supervisions • Evaluate effectiveness • Service User feedback • Support Worker feedback • Goals achieved
