Objective and Measure Provide patients with timely electronic access to their health information (including lab results, problem list, medication lists, medication allergies) within four business days of the information being available to the EP. At least 10% of all unique patients seen by the provider during the EHR reporting period are provided timely (available to the patient within four business days of being updated in the certified EHR technology) electronic access to their health information subject to the EP's discretion to withhold certain information.   6. RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: one or more face-to-face visits with the provider (Service Category of A, S, O, or M) during the EHR reporting period Patient Electronic Access

  7. RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the denominator WHEN: EP answers “Yes” to having access to the Personal Health Record (PHR) Patient Electronic Access

  8. RPMS Reporting Logic Measure Exclusion: • A provider who neither orders nor creates lab tests or information that would be contained in the problem list, medication list, medication allergy list (or other information as listed at 45 CFR 170.304(g)) may be excluded from this measure and will have to attest to this in separate documentation to CMS. The MU Report will not take any potential exclusion of this measure into account. Patient Electronic Access
