ServiceSource/NVTC • ServiceSource has been transitioning individuals to community since 1974 • Serving 51 mutual participants when the Settlement Agreement was signed in 2012 • Since the signing, 33 mutually served individuals have been discharged from NVTC: •  18 relocated to a residence out of their home community – close to 60% • 11 have remained with ServiceSource post discharge and are in a combination of ICF and Waiver group homes • 2 deceased • 2 relocated to nursing homes (1 local / 1 out of area) • 18 remaining participants awaiting discharge from NVTC • ServiceSource commits to services for those moving to a local group home who wish to remain in our program (despite funding gap) 8

ServiceSource/NVTC: Partners since 1974 • Challenges to serve NVTC exiters • Lack of local residential capacity due to inadequate Waiver rates • Upon discharge, sponsorship is reduced 40% from Medicaid ICF to Medicaid Waiver rate • Continue to serve or expand capacity funded at less than 60% of cost • No Waiver reform prior to scheduled closure • Recommendations for a safe, effective closure • Regional Service Rate (CSBs, Community ICFs, DARS) vs Medicaid Waiver rate • Increase availability of ICF model • Realistic Waiver reform that equates sponsorship with locally vetted, contracted and sponsored rates 9

Re-Designed Organizational Employment model • Re-designed traditional center based model • Stakeholder involvement • Decentralized smaller community settings • Multiple locations • Moves away from organizational employment model to person selected programming array • Augmented services replacing organizational employment model include; • Employment in community businesses • Support for entrepreneurial businesses • Day support for those with significant needs • Expressive therapies • Volunteer options • Health and Wellness

Disability Resource Center • Disability Resource Center opened in December 2014 • Career Center • Drop in center to receive job placement support and guidance, access to computers to submit resumes and complete applications. • Wellness Support Group, Benefits Counseling with vaACCSES, Monthly Workshop with Career-Related Topics • Houses Specialty Programs – Autism, Family Support, Warrior Bridge • Community Partnerships – provide space to mission-similar organizations • Assistive Technology Lab to provide AT assessments in partnership with GMU • Blossom Daily Café and Gift Shop • Re-designed Employment Resource and Community Integration Center (ERCIC)
