  1. Introduction The Sherbrooke Questionnaire is one of the proposed case finding tools for the Single Assessment Process (SAP) Patients trigger the next step in the SAP (Overview Assessment) by scoring 2+ on the Sherbrooke Questionnaire We evaluated the feasibility of using this tool in primary care. Methodology All home dwelling patients aged 70 and over registered with one primary care practice were sent the Sherbrooke Questionnaire. We re-mailed those who did not respond within 4 weeks. Those who failed to answer after a further 4 weeks were deemed non-responders. The Sherbrooke Postal Questionnaire (with positive triggers) Do you live alone? Yes/No Do you take more than 3 different medications every day Yes/No Do you use a stick, a frame or a wheelchair to move about? Yes/No Do you see well? Yes/No Do you hear well? Yes/No Do you have problems with your memory? Yes/No Results Number of questionnaires sent out 945 Total number responding 853 (91.3%) Responded to first mailing 819 (86.6%) Responded to second mailing 44 (4.7%) Not known at address or recently died 24 (2.5%) Non responders 58 (6.1%) 0.1% short Patients who triggered an Overview Assessment 564 (59.7%) Questionnaires with a score of 2+ 484 (51.2%) Questionnaires with missing data 22 (2.3%) Non responders 58 (6.1%) 0.1% short Conclusions The Sherbrooke Questionnaire sent to patients by their own GP yielded an extremely high response rate of 91.3%. 59.7% of this targeted population triggered further assessment within the SAP It is feasible to use the Sherbrooke Questionnaire as a case finding tool for the Single Assessment Process within General Practice.
