End Date • Position # • Hourly Rate • Fund TypeSelect: State or FoundationEnter Six Digit Fund # and Acct. #Provide estimated salary • Route for Signatures Do not SAVE any documents due to modifications.

Employees The following documents are required (see below): • I-9 • W-4 • NC-4orNC-4EZ • Direct Deposit Form • Criminal Background Check • Race & Ethnicity Information Sheet • FOR PREVIOUS TEMP. Step #1401 • Be as specific as possible when stating the reason for revision. • ACCEPTABLE forms of revision should include either ALL signatures OR only the signatures of the Originator and Human Resources Personnel • When revisions are established please notify: • HR – Maggy Sivansay • Budget – Jackie Riffle • Payroll – Debra Gunter Please include the word “REVISION” in the subject line of your email when sending notifications
