Writing the Service Design is one of the most daunting tasks in starting your Supported Living Agency. The Service Design Template for California Supported Living can save you months of research and work. The Service Design Template holds all the areas that need to be included by most regional centers, though this is geared toward San Andreas Regional Center. Many of the areas are populated with information that there is no reason to rewrite; other areas prompt you with specific and directed questions that fit the criteria for that area. Examples are offered throughout and are there for modification and adaptation to your agency and son/daughter. The 40 Table of Content line items help reduce the likelihood of that you wont skip a beat. This tool may be purchased for $129.00 from Journey of Choice, Inc. for the sole purpose of a single use by the purchaser to create a service design for one supported living agency using Title 17 as the driver and guide. I recommend that you make a blank copy of the document before you start as a back up so that if you make a huge error, you have the original. Save as you go. There is nothing more frustrating than making great progress and then losing it all. I often save it in versions as I go along. That way I have the original version of the writing, then a new copy with the new changes. Servicedesignv1.doc Servicedesingv2.doc and so on Do what works for you.
