Change Change, change and more change, Of context, place and time. Why is it that Life’s transient stage Plays havoc with my mind? You said “we’ll go to McDonalds” But this was just a thought. I was set for hours, But the plan then came to naught
My tears and confused frustration, At plans that do not appear, Are painful beyond recognition, And push me deeper into fear. How can life be so determined? How can change be so complete? With continuity there is no end, Security and trust are sweet.

Year 5- Spring Term • Contact multi-agencies to arrange assessments. • Arrange date with agencies for Y5 annual review or school meeting.
Year 5 – Summer Term • Attain pupil’s views on their progress and any transition concerns. • Share pupil’s views at beginning of Y5 review. • Undertake Y5 review. • Primary SENCO to provide leaflets for family on transition.
Year 6 –Autumn Term • Arrange visit with pupil and parent to Secondary school at end of day.
Year 6 – Spring Term • Arrange for visit to Secondary school with LSA part way through an ALN lesson and leave before end of lesson. • Arrange another visit to experience lesson transition (from a safe view point). • Secondary SENCO and Head of Y7 to meet Pupil at Primary school.
Year 6 – Summer Term • Develop pupil passport with pupil. • Invite relevant agencies and Secondary SENCO to Y6 review meeting. • Pupil to complete ‘what I need for Secondary school’ sheet. • Undertake Y6 review. • Forward Pupil Passport to Secondary school. • Secondary school to provide timetable before summer holidays. • Primary teacher to complete student summary form and send to Secondary SENCO.
Year 7 • Close monitoring by form tutor/Year Head/LSA and SENCO. • Buddy system. • Time out card. • Clear school rules. • Consistent routines. • Year Head to be available first half term to address parents concerns.
Transition Transition is about moving, to where or what one asks? This is my very question, from present or the past. Time for me is all the same, I know not of its future I only know I trust in now…..tomorrow can come, I only need to know how. (Wendy Lawson, 2003)
