Written Consent Required at all households visited Separately for everyone tested. Consent happens once at the beginning of the household visit for the person answering the PDA questions.
Consent Process • Introduce yourself to head of Household • Explain study to Household head • Read consent document to Household head • If agrees to participate have them sign or thumb print consent document and proceed with testing. • Keep signed copy • If refuses try and explain further if more clarity needed, supervisor maybe asked to assist • Leave copy of consent document if requested
Consent Process continued The consent covers the questions which will be asked about the household on the PDA as well as the testing and treatment procedures.
Consent for Under 18yrs Only adults over age 18 can give consent. Anyone under age 18 must have someone over age 18 give consent for them. Anyone under age 18, they will also need to have the process explained to them and we have a separate oral assent form them, although it does not require any signature
Consent Process Summary (Under 18yrs) After Head of Household ask HH head for consent to test those below 18 yrs available in House hold Retain all signed consent forms
All above 18 yrs need to be asked for consent separately Collect all signed consent forms
Above 18yr Olds Consent • Have each person above 18yrs sign or right thumb print Statement of Consent • Have a witness sign the Statement of Consent form for each Household member above 18yrs.
For the signing part, there are 2 questions to go through to affirm or decline. It is ESSENTIAL to answer these in the PDA YES or NO as well as on the consent document. (Insert PDA shot) Consent Document
Consent Process Reminders • Ask if they want a copy of the information component of the consent and you should be prepared to leave them a copy if they want it. • CHW/Enumerators need to retain the signed copy. • All copies of signed consent forms should be kept and delivered to the supervisors.
