  2. Total severity 4 and 5 observation for the month of July.’10:- 35 Nos.

  3. After Before Severity 4: – Electrical cables were passing from logged rain water. Siemens Fender Kharagpur (East). Action Taken:Cable stand provided.

  4. After Before Severity 4: Portable Grinder without guard. Siemens Fender Kharagpur (East). Action Taken:Wheel guard provided.

  5. Before After Severity 4: Wooden electrical Panel box with domestic socket found in use at site. Siemens Fender Kharagpur (East). Action Taken:Metalic panel box with industrial socket provided.

  6. Before After Severity 4: Joint in electrical cable was covered by PVC tape only. Siemens Fender Kharagpur (East). Action Taken:Male-Female plug provided for electrical joints.

  7. Severity 4: Worker was fixing lifeline on the erected rafter anchoring his layanyard to the lifeline post. CSP (East) Action Taken: Work stopped. Boom-lift provided for the fixing of lifeline rope in the lifeline post.

  10. Severity 5 – Riggers & Supervisor found w/o Height safety Precaution for Walkway work. (Sun Pharma-Halol) (K.J.IRON) _West (G) Action Taken: Warning given to contractor Site In-charge & supervisor.

  11. Severity 5 – Life line was removed w/o completing secondary work. (Sun Pharma-Halol) (K.J.IRON) _West (G) Action Taken: Warning given to contractor Site In-charge & supervisor.

  12. Severity 5- Nylon Lifting belt was found damage at site. (Jyoti Plastics- Valsad.) (R.K.Builder) _West (G) Action Taken:- Instructed site In-charge Dis-card it immediately .

  13. Severity 5- OneGas Cutting Set was found w/o Flash back arrestor at Cylinder end.(Welspun- S.K. Engg.) _West (G) Action Taken: Stopped activity & Flash Back arrestor provided Immediately.

  14. Before After Severity 5- . Industrial Plug tops not provided. (Welspun – S.K. Engg.) _West (G) Action Taken:- Work Stopped and Immediately Industrial Plug top provided..

  15. Severity 5- Scaffolding was erected w/o ladder, Fall arrestor & Short Platform. (Welspun- S.K. Engg.) _West (G) Action Taken: Work was stopped and asked builder Staff to provide Ladder, Fall arrester & 500mm Platform immediately (in process).

  16. Severity 5-Electrical wire not routed properly & Electrical Box (Industrial Plug top) & ELCB/RCCB (30mA) not provided. (Bafna Nasik -Prathmesh.) _West (M) Action Taken:- Work stopped ;Asked builder Staff to provide ELCB/RCCB/MCB immediately.

  20. Severity 5- Non-std Single pole Scaffolding Erected. (Krishidhan- Jalana.) (DEVMATA) _West (M) Action Taken:- Instructed them Non Standard Scaffolding Dismantle immediately . (Repetitive violations: Cancelled awarded order for next project)

  21. Severity 5- MCB installed instead of RCCB. (Krishidhan- Jalana.) (DEVMATA) _West (M) Action Taken:- Instructed them to Provide RCCB immediately (Done)

  24. Before After Severity 5- : Primary member erected without lifeline post (Perlos, Chennai) Action Taken:- Necessary fall protection arrangement provided (Perlos, Chennai)

  25. Severity 4- : No proper lifeline post and no safety harness not harness secured properly at heights (Sheenlac, Chennai) Action Taken:- Work has been stopped (Sheenlac, Chennai)

  30. Severity 5- : Damaged electrical panel board and damaged harness (Cryolor Asia,TN) Action Taken:- Stopped the activity and rectified (Cryolor Asia, TN)

  31. Severity 5- : No proper access, Workers not having safety harness while working at Height (VRV, Tada) Action Taken:- Work has been stopped for necessary compliance (Done)

  34. Severity 4 – Scaffolding being erected on tractor( Carls berg , Hyderabad) Action Taken : Stopped work immediately and instructed to dismantle the structure safely

  35. Severity 4 - Area not proper for crane movement ( Carls berg , Hyderbad) Action Taken : instructed supervisor to coordinate with civil supervisor ad make the area leveled.

  36. Severity 4 –Air compressor without guard ( Carls berg , Hyderabad) Action Taken : Instructed supervisor to provide guard.

  37. Severity 4 –Worker found working on the scaffolding without wearing safety harness( Carls berg , Hyderabad) Action Taken : Instructed supervisor to ensure that all workers are wearing work related PPE.

  38. Severity 4 –Girts and guide ropes not tied as per the procedure( Carls berg , Hyderabad) Action Taken : Instructed supervisor to girts and guide ropes

  39. Severity 4 –Workers found working on the rafter without any fall protection( Carls berg , Hyderabad) Action Taken : Instructed supervisor to provide lifeline on the rafter
