  2. Our background check process with NCSI is in place to ensure consistency, credibility, and professionalism at our events. Passing the background check allows National Sports M.A.P. the confidence to actively pursue work in your community with your skill sets in mind. The cost is only $58 and lasts for 2 years with a re-check at the 1 year anniversary date.

  3. So now you are probably thinking: “but I already passed a background check!?” We understand that many of our practitioners have already completed & passed a background check. However, in order to bring together practitioners of all types for all our events, we need assurance that each practitioner we work with are Certified, Licensed, and cleared of any criminal history prior to working any event.

STEP 1: NCSI Background Check Process Go to NCSIsafe.com *Step 1A: Enter Self Registration Number: 44550385                                                                                                               License or Certification Verification Program*Step 1B: Enter Self Registration Number: 66438142      Education Verification Program The background check consists of the following required Human Resource documents: $58 payment to NCSI Completed NCSI Background Check Documentation National Sports M.A.P. Standard Operating Procedures National Sports M.A.P. Terms of Use Please pay special attention when reading & acknowledging the information.

STEP 2: National Sports MAP Registration Process Register at www.nationalsportsmap.com & complete your profile in detail • Name, Email, Phone Number, Location, Website • License, Certifications, Insurance information • Post a professional profile picture • No logos or non-practitioner photos will be approved • What sports and types of events do you have experience working with?

  6. STEP 3: National Sports M.A.P. Registration Process Please email the following documents to: info@nationalsportsmap.comSubject: Last, First Name Practitioner Docs (A) First Aid, CPR, AED Certification Cards (B) License or School Certificate & Insurance Information** **If no License is required in your State, please send in your Certificate Information. **Independent Practitioner Professional Liability Insurance is mandatory.

  7. Fast Track Communication with National Sports M.A.P. Once your background check is approved  we will send you a welcome e-mail from Brian.Keene@nationalsportsmap.com • This e-mail will contain important information including but not limited to what we offer &aW-9 form for payment completion • Please respond via e-mail with the completed W-9 & any questions you may have after your review
