General Information • Environmental, Health, Safety and Risk Management (EHSRM) • Promotes and supports a safe and healthy campus environment • Offers consultation and training in environmental, health, safety and risk management • Establishes policies and procedures as they relate to EHSRM • Investigates unsafe conditions, accidents and injuries • Applies for and manages state and federally required plans and permits • Is the main contact for all state and federal regulators and inspectors

4. Occupational Safety • Assisting you with Occupational Safety and Health Administration (OSHA) requirements and other recognized health and safety directives. Ways we can assist: • Safety Assessments: Provide an on-site safety evaluation of your workplace • Accident/Incident Investigations: Determine root causes and provide corrective action provisions to help prevent similar accidents/injuries • Unsafe Condition Report Program: Helping our UAF community remain safe by identifying and correcting campus conditions that could result in injury • Safety Awareness Incentive Program: Rewarding employees for voluntary participation in selected safety programs which ultimately aid in compliance

Occupational Safety • Ice Cleat Program: Providing UAF employees with free ice cleats (traction devices) to help prevent slips and falls on ice/snow • Remote Travel Emergency Plans (RTEP): Assisting in developing and reviewing UAF RTEPs which help ensure a safe trip afield • Job Hazard Analysis (JHA): Providing specialized training and assistance to develop JHAs which identify workplace hazards and protective measures

6. Occupational Safety • Safety Awareness Incentive Program: Rewarding employees for voluntary participation in selected safety programs. Some of the ways employees can win incentive prizes include: - Completing all required safety training - Answering the “Safety Challenge” question on our website - Submitting an “Unsafe Condition Report” from our website - Just by being observed working in a safe manner Remember, you can NOT be released from your job, reprimanded, or otherwise disciplined, for reporting a safety concern. 7. Accident and Injury ANNUAL AVERAGES • $ 625,000 • 85 RECORDABLE INJURIES • 32% ARE SLIPS, TRIPS, FALLS RECENTLY • 2012 - 146 INJURIES • 2013 – 127 INJURIES THROUGH JULY “Go ahead and laugh. Snow, sleet, or ice, I have not fallen once in 5 years.”

9. Accident – Injury Reporting • Report any instance of personal injury, regardless of severity • Examples include: cuts/lacerations, injuries from slips and falls, chemical exposures, allergic reactions, muscle strains, repetitive motion/ergonomic injuries, debris in eyes • Employees must use the UAF Accident/Incident Report found at http://www.uaf.edu/safety/ • DO NOT use the UA Incident Report on UA’s website • Complete the report and send to EHSRM within 4 working hours of the injury event • Report student and visitor injuries directly to the System Office of Risk Services Claims Department at 907 786-1716

10. Accident – Injury Reporting • Call 911 when injuries require prompt medical treatment • Immediately report to EHSRM any accident with injuries that results in transport for medical treatment or results in known or potential overnight hospitalization • Overnight hospitalization mustbe reported to EHSRM no later than 8-hours from the time the accident occurred Failure to notify EHSRM of an overnight hospitalization within 8-hours of the accident can result in OSHA citations and penalties (fines) to the department!

11. Accident – Injury Reporting Worker Compensation Claims • Claims should be filed for any OSHA recordable injury • Effective 07/22/2013, all Alaska employers/employees MUST use the new Alaska forms to report a claim of work injury or illness • Each reporting of injury/illness now requires two forms: • Employee Report of Occupational Injury and Illness to Employer (Form 07-6100) http://www.uaf.edu/safety/ • Employer Report of Occupational Injury and Illness to Division of Worker’s Compensation (Form 07-6101)Forms must be completed within 10 days of accident http://www.uaf.edu/safety/ • Completed forms must be printed, signed, and faxed to the System Office of Risk Services (907-786-1412) and UAF EHSRM (907- 474-5489)

Environmental • UAF is regulated by state/federal agencies; we have the following permits and plans • State Air Quality operating permit – main campus • State Stormwater Discharge Permit and Plan– main campus • Federal Spill Prevention, Control, and Countermeasure Plan (SPCC) for oil spills – all campuses and facilities • GHU Wastewater Discharge Pretreatment Plan – main campus, Lena Point, UAF hanger • Federal 90 day Hazardous Waste Accumulation Point permit – main campus • Federal Wetland Permits – project specific

Risk Managementhttp://www.uaf.edu/safety/risk-management/ • Identify, assess, and manage risks on and off campus • Volunteer Approval Process: review and approval for all volunteers on and off campus • Travel Evaluations and Recommendations: abroad, remote, chartered planes and boats, with family, friends, students, volunteers or employees • Event & Student Activity Reviews: summer camps, graduation, annual log show, races, Red Green Regatta, Pub Crawl for both UAF and non-UAF users. If you believe there may be some risk involved in your activity you should contact risk management • Program Risk Assessments: identify potential risks, evaluate, and find ways to manage risk by controls, modifications, or by transferring the risk through insurance or contract/waiver language

Cont’d • Loss Prevention Program: approval of funding for initiatives that reduce our potential losses through accidents or injury to people or other incidents that may cause damage to property or equipment • Protection of Minors: implementation of policies and guidelines; conducting risk assessments on and off campus for events, activities, summer sessions, and camps for minors (individuals under the age of 18) • Reviews and updates contracts, agreements and waivers • Contract Reviews for procurement of services, grants and research funding, independent contractors, facility use agreements, and Memorandum of Agreements
