Family Care • Through the managed care model, the Family Care benefit is available to eligible persons who are frail elders, or who have a physical or intellectual disability.
Family Care • An independent evaluation of the Family Care program found that Family Care saved an average of $452 per member per month by helping people stay healthy and more independent in their homes and communities.
Statewide Expansion • In February 2006, Wisconsin Governor Jim Doyle announced his goal to expand the Family Care program statewide over the next 5 years.
Personal Outcomes • “I want to stay in my own home.” • “I want to find a job working with animals.” • “I want to start a business.”
Employment Outcome • “I want to work………” • Meet with your Family Care team, family and friends to create a plan to meet your goal.

Exploring SDS and Employment CCCW received an Integrated Employment Grant from Pathways to Independence….. develop a comprehensive system that maximizes employment for people with disabilities, increases the state’s labor force through the inclusion of people with disabilities, and protects and enhances workers’ healthcare, other benefits and needed supports.
